Maintenance Program with Coaching — $129/month

What's Included:

1 bottle of Maintain4Life™ each month
1 one-on-one coaching session per month (30 minutes)

Coaching Sessions: Call the office at (779) 201-1225 to schedule your monthly 30-minute appointment. Sessions must be scheduled and completed within the same calendar month they are billed. Unused sessions do not roll over and are not refundable.

Product Delivery: You may pick up your bottle in-office at no additional charge, or have it shipped to you for a $25 shipping fee added to each monthly charge.

Billing & Auto-Renewal: This is a monthly auto-renewing subscription. You will be charged $129 automatically each month on the payment method you provide, and that method will continue to be billed monthly until you cancel.

Cancellation: To cancel, please call the office at (779) 201-1225 and speak with a team member at least 30 days before your next billing date. Cancellations cannot be processed by voicemail, email, or text.
